What is an IMM5990 Form?

The Statutory Declaration to Confirm the Relationship of an Extended Family Member, known as form IMM5990E, is part of Canada’s new immigration initiative for humanitarian grounds and provides a pathway for Colombian, Haitian and Venezuelans nationals with family in Canada, to come to Canada.

This form confirms the extended family relationship between a person applying to come to Canada, and their extended family member already residing in Canada. All family members within Canada that sign the form will need to be either a permanent resident (PR) or Canadian citizen.

The form also confirms that the family member will act as an “Anchor” and support the applicant coming to Canada for one year.

This program was signed and launched on November 17, 2023, will have availability for 15,000 applicants.

Why is the IMM5990 Form needed?

With this program, applicants will need to fill out a statutory declaration required to be signed by a notary public and is to be completed by individuals who seek to apply through the Permanent Residence pathway for Colombian, Haitian and Venezuelan nationals. Applicants must meet the eligibility requirements and plan to settle permanently in Canada, outside the province of Quebec.

Who can fill out the IMM5990 Form

The IMM5990 form can only be signed by a permanent resident (PR card holder) or Canadian citizen. The individual will also need to be an extended family member of the applicant, and act as an “Anchor” upon their arrival to Canada.

  • You are Colombian, Haitian or Venezuelan national
  • Be located in Mexico, Central or South America, or the Caribbean
  • Have a spouse, common-law partner, child (regardless of age), grandchild, parent, grandparent or sibling of a Canadian citizen or permanent resident who is willing and eligible to support your application (an “anchor”)
  • Plan to live in any other Canadian Province outside Quebec
  • Have a Government issued passport or eligible travel or identity document

Your immediate family members (dependent child under 22, spouse or common-law partner) may be included in your application.
