This form must be completed by a Canadian citizen or permanent resident living in Canada, known as the “anchor”. It must also be sworn or affirmed and signed before an official authorized to receive a solemn declaration.
A qualified national must meet the eligibility requirements and plan to settle permanently in Canada, outside the province of Quebec.

The Purpose of an IMM 5990 Statutory Declaration
In 2023, the Government of Canada opened a dedicated humanitarian pathway to provide permanent residence to Colombian, Haitian and Venezuelan nationals. This pathway is called the Temporary public policy to facilitate the issuance of permanent resident visas to certain Colombian, Haitian and Venezuelan nationals with family in Canada.
This means if you’re a Canadian citizen or permanent resident in Canada, you can support extended family from Colombia, Haiti or Venezuela coming to Canada.
As the anchor, you need to complete the IMM 5990 Statutory Declaration to help your family use this pathway. This form demonstrates that you meet the criteria required to adequately support your family in settling into Canada.
How to Complete an IMM 5990 Statutory Declaration
Once you’ve gathered all the necessary information, completing the IMM 5990 Statutory Declaration is simple. As the anchor, you’re required to complete the entire form, including the sections asking about the foreign national and their family members. Follow the instructions below to have your form ready in no time.
Section A: Your Personal Details (Canadian Citizen or Permanent Resident)
Enter the following information to complete Section A:
- Your full given name(s) and last name, as shown on your passport, travel document or identity document
- The applicant’s unique client identifier (UCI), provided by IRCC (if known)
- Your place of birth, including the city or town and country, as shown in your passport or travel document.
- Your date of birth
- Your status in Canada (Canadian citizen or permanent resident)
- You must provide a photocopy of your document to confirm your Canadian citizenship or permanent resident status
- Your residential address
- Your telephone number (including the country code, area/regional codes)
- Your email address
Note: If you do not know the UCI or travel document number, leave those fields blank. If you don’t know any dates of birth, enter 01 to fill in the spaces for the unknown year, month or day.
Section B: The Foreign National’s Personal Information
Enter the following details to complete Section B:
- The national’s full given name(s) and last name, as shown on their passport, travel document or identity document
- Their unique client identifier (UCI), provided by IRCC (if known)
- Their place of birth, including the city or town and country, as shown in their passport or travel document.
- Their date of birth
- Their citizenship status
- The type of identity or travel document they hold
- Travel document number (if known)

Section C: The Accompanying Family Members’ Personal Details
Enter the following details to complete Section C:
- All of the foreign national’s accompanying family members
- Each of their last names, as shown on their passports, travel documents or identity documents
- Each of their full given names (including first, second, and more) as shown on their passports, travel documents or identity documents.
- Each of their dates of birth

Section D: Anchor Eligibility Assessment
Enter the following details to complete Section D:
- Answer each question by checking the appropriate box
- If you answered “Yes” to one or more of the questions (5-14), you need to explain what happened in the allotted space

Section E: Country of Residence
Enter the following details to complete Section E:
- List all countries you’ve lived in for 6 months or more since you were 18 or over the last decade, whichever is most recent.

Follow the steps below to complete the declaration portion of your form. You should only complete this section in the presence of an authorized official. Do not sign this portion before meeting with the official.

- Check the box to indicate the relationship between yourself and the foreign national
- If you check “other”, you must specify the relationship
- Only sign and date this portion in the presence of an authorized official, like a notary public
- By signing, you officially declare that you understand the questions asked, and that the details you’ve provided are complete, truthful, and correct.
- Your signature also confirms that you’ll support the person identified in Section B and their accompanying family members.
Section G: A Canadian Authorized Official’s Declaration
A person who is authorized to receive a solemn declaration by law must complete and sign this section.

Which Family Members Can Act as an Anchor?
To support a loved one through this pathway, you must be one of the following to the foreign national applying to enter Canada:
- Common-law partner
- Child (regardless of age)
- Grandchild
- Parent
- Grandparent
- Sibling

Who Can Apply for Permanent Residence Through This Pathway?
For a foreign national to be eligible to apply, they must meet the following criteria:
- Be a Colombian, Haitian or Venezuelan national
- Be located in Mexico, Central or South America, or the Caribbean
- Be a spouse, common-law partner, child (regardless of age), grandchild, parent, grandparent or sibling of a Canadian citizen or permanent resident who is willing and eligible to support your application (an “anchor”)
- Plan to live outside Québec
- Have a valid passport or eligible travel or identity document
Immediate family members of the foreign national (dependent child under 22, spouse or common-law partner) can be included in the application.
