Frequently Asked Questions
What is an OSAP Affidavit of Indigenous Identity?
An OSAP Affidavit of Indigenous Identity is a sworn document confirming that you identify as Indigenous for your OSAP application. OSAP may request this Affidavit if it cannot verify your Indigenous identity using standard supporting documentation.
The Ontario Student Assistance Program (OSAP) may ask for this Affidavit if you cannot provide one of these documents to show Indigenous identity:
- Secure Certificate of Indian Status (status card)
- Temporary Confirmation of Registration Document (TCRD)
- Inuit enrollment or beneficiary documentation
- Métis registry or citizenship documentation
- Letters or confirmations from recognized Indigenous governing bodies or organizations
You may be asked to submit this Affidavit if your situation falls into one of the following categories
- You identify as First Nations, Inuit, or Métis, but don’t have formal documented proof.
- Your Indigenous documentation is delayed, expired, lost, or being replaced.
- Your name or personal details don’t match across your OSAP application and supporting documents.
- You’re adopted or have limited access to family records.
- OSAP flagged your application to confirm your Indigenous identity
Note: If OSAP cannot verify your Indigenous identity, funding may be reassessed, and you may lose access to Indigenous-specific OSAP grants or funding.
Do you have Questions about your OSAP Affidavit of Indigenous Identity Document ?
What is an OSAP Affidavit?
An OSAP Affidavit helps you complete your OSAP application and secure financial assistance for your studies. You might need one if you lack the documentation OSAP requires to prove a certain status in your OSAP application.
This Affidavit contains a written statement of facts provided by your spouse whose income is being reported. Your spouse must swear or affirm that the contents of the Affidavit are true and sign it before an authorized official, such as a notary public or commissioner of oaths.
Providing false information to OSAP may result in penalties under provincial student-aid rules. These can include reassessment, repayment, interest charges, or restrictions on future funding.
Signing an OSAP Affidavit with a Notary Public
In Canada, Affidavits (including your OSAP Affidavit) must be commissioned to be considered valid. This is a necessary step, and without it, your Affidavit is not valid.
You can meet with a notary online to have your Affidavit commissioned in 7 minutes, from anywhere with a Wi-Fi connection.
Commissioning an OSAP Affidavit: Step by Step
You can commission your Affidavit online over a secure video appointment. During your online notary appointment, the following will take place:
- You’ll present a piece of valid, government-issued ID to verify your identity.
- You’ll swear or affirm that the contents of your Affidavit are true.
- You’ll confirm that you understand the document and are signing voluntarily.
- You’ll sign and date the Affidavit while a notary witnesses in real time.
- A notary signs and applies their official stamp or seal.

Note: Knowingly providing false information may be considered perjury under Canada’s Criminal Code.
The Importance of Confirming Indigenous Identity
All students who apply for OSAP funding are generally expected to make a fixed student contribution toward their studies. If you identify as Indigenous and provide documentation to confirm your Indigenous identity, you may be exempt from this fixed student contribution. You may also qualify for additional grants and loan funding for Indigenous students.
