OSAP Affidavit of Sole Support Parent | Free Template

In Ontario, you must be separated, divorced, widowed, or single to be considered a sole support parent. You also need to have a dependent child (or children) living with you for at least 50% of a study period. Dependent children can be your natural, adoptive, or foster children. Each child must typically be under 18.

If a child is 18 or older, they must meet one of the following conditions:

• Be enrolled in high school with a minimum course load of 60%
• Be enrolled in a full-time college or university program and out of high school for under six years
• • If they’re attending school abroad, you must be the parent the child lived with last
• Have a disability and be wholly dependent on you

You may need to OSAP Affidavit of Sole Support Parent if you cannot provide the documents listed above. This can happen if you:

• Are separated informally without a written Separation Agreement
• Share custody without a formal court order
• Recently separated or divorced, and documentation isn’t yet finalized
• Never went to court to establish custody or child support
• Were asked to provide sworn confirmation during OSAP verification

You don’t need to have sole custody of your child to be considered a sole support parent for OSAP purposes.

Note: If you receive child support from your ex-partner, you can still qualify for sole support parent status.

Commissioning an OSAP Affidavit: Step by Step

• Personal information, including your full name and current permanent address
• The names and dates of birth of the children
• Confirmation that your child(ren) will live with you at least 50% of the time during the academic year
• The date of your separation or divorce (if applicable)

• If you are widowed, you must also provide a copy of your spouse’s death certificate.
